Enrollment Procedures
- Contact our office either via email or by phone to request information.
- Download the intake packet or we will send you an intake packet with information about our programs as well as various applications and forms to be completed.
- Once we have received your completed packet in the mail we will call you to set up an initial visit.
* For your convenience we do bill insurance and accept Medicaid for therapy services.
